Developing early testing of improved neonatal care and follow-up pathways for babies and families at increased risk of adverse outcomes.
Working with local communities, hospitals and health partners, this theme is developing and testing better care pathways for newborns and young children to reduce health inequalities. It focuses on two high-risk groups: babies born into socially disadvantaged families, and medically vulnerable babies needing extra support after leaving hospital.
Both groups often struggle to access joined-up care, discharge support and early help leading to poorer health outcomes, developmental delays, unplanned hospital visits and lower parental wellbeing.
